Offering Pilates-based physical therapy in Royal Kunia, Mon-Fri from 8am-2pm.

I accept Tricare insurances with an authorization faxed to 808-748-0162. For all other insurances, self-pay is available. Per request, I can provide a super-bill that you can submit to your insurance company for potential out-of-network reimbursement. This would require a physician's referral faxed to 808-748-0162.
